The forms in this section are used by the Title III team members and the Selection Committee to prioritize, track, and
organize development of Title III projects and use of Title III funds. The forms are available in Microsoft Word and
Adobe PDF formats. To submit a request form, download/open the appropriate form below, complete the form and return
it to the DE department.

Complete this form when you are requesting funding and support for the development of a full-fledged online course. Five courses per year of the grant will be selected by the selection committee and faculty will be awarded a $2000 stipend for those courses that meet the criteria listed in the documentation section of this web site. Title III Mini-Grant Proposal Complete this form when you are requesting support for an instructional technology focused project.
